Register Your Child for Baptism

To register your child for the Sacrament of Baptism you must meet the following:

• You must be a registered Active & Participating member of Saint Monica's for a minimum of three months prior to registering to have your child baptized.
• You and your spouse must attend a Baptismal Preparation Class.   
• Godparents  must be actively participating Catholics.  
• We require a letter from their parish indicating that they are qualified to act as Baptismal Sponsors.

The following information is required for the Sacramental register and to complete a Baptismal certificate.  Please complete the entire form.    Once this has been submitted to our office you may call to schedule your Baptism.
